Kevin Wendell Crumb (James McAvoy) has 23 toothbrushes in two cups on the shelf in his room at the mental institution, meaning that each of his personalities has their own toothbrush, with the exception of The Beast who apparently doesn't brush its teeth.
The sound of running water heard during David's entrance scene was him washing his own hands beforehand to make sure that he doesn't kill the teenagers with his strength by accident.
